The New Hampshire Care Collaborative’s mission is to empower, educate, and engage people in our communities to identify and connect with local services and supports to live an independent, dignified, full life.

About Us

The heart and soul of NH Care Collaborative lies in the collective vision of a group of non-profit leaders in the Monadnock Region known as the Monadnock Partnership. Over twenty years ago, with the support of the Monadnock United Way and the Robert Wood Jonson Foundation this group created a shared intake, assessment, and referral process for citizens of the Monadnock Region, as well as an interagency cross training system for social service and health care providers, referred to as the Community Network Team. These agency leaders recognized that despite the great work of each of their organizations, people were falling through the cracks in the complex health and human services system and that social service agencies needed to work together in a deliberate, collective way to cross train staff and to provide complex care coordination for people who need an extra hand to access services. They dedicated themselves to creating an interagency process to help people regardless of age, ability, or financial means.

In 1998 and 1999

Five of the Monadnock Partnership organizations established two formal entities with a shared Board of Directors based on their shared Monadnock Partnership vision and principles. The first was a Limited Liability Company (LLC) called Pilot Health, which helped to develop a new NH Medicaid waiver program, now called Choices for Independence (CFI), and the second, a non-profit 501c3 called the Monadnock Collaborative, to house one of NH’s newly established, federally designated Aging and Disability Resource Centers (ADRC), now known as ServiceLink. The Monadnock Collaborative grew as it developed the following programs, Dental Health Works, Monadnock Voices for Prevention, and Caregiver Connections, and expanded its core programs to other parts of the state. Now the two original organizations, Pilot Health LLC and the Monadnock Collaborative have merged. The LLC has been dissolved and the programs operate under the Monadnock Collaborative. In order to better recognize the statewide reach of the organization and its work with community, state, and federal partners, the organization rebranded in 2021 as The New Hampshire Care Collaborative. 

Today

NH Care Collaborative continues its work of improving service coordination and streamlining access to care with the added benefit of a website and use of technology to enhance the individualized, in-person connection and support it offers citizens across the state.
