Aging, Disability and Endless Paperwork

It’s figuring out where to start getting help.

One phone number leads to another.

Help consists of just a referral to yet another agency.
Applications feel impossible.
No one explains what benefits exist or how they work together.

Before long, people feel like they’re failing, when really, they’re stuck in a system that was not designed for convenience.

That’s where the Aging and Disability Resource Center (formerly ServiceLink) comes in.

Located at 105 Castle St. Keene and 33 Arbor Way in Charlestown, NH, NH Care Collaborative houses the Aging and Disability Resource Center (ADRC), a free program designed to work alongside older adults, people with disabilities, caregivers, and families to help them understand their options and create a plan that feels doable.

At NH Care Collaborative, the message to clients is simple:  “Our job isn’t to add more to your plate. Our job is to walk beside you, translate the system, and provide options so you can make the best decisions for your unique needs”

Taking the mystery out of social services

Every situation is unique, but the questions are similar:

  • What services are available?
  • Do I qualify for financial help?
  • How do Medicare, Medicaid, and long-term care work together?
  • What resources exist for aging at home?
  • Who can help me advocate when I don’t know what to ask?

Instead of giving families a referral and sending them off to navigate the system on their own, the ADRC programs help coordinate information and connect people to the right programs, agencies, and supports.

Sometimes that means reviewing paperwork.
Sometimes it means making calls together.
Sometimes it means simply listening, then helping someone take the very next step.

  • Professional and informed — guidance is rooted in knowledge of New Hampshire’s systems and supports.
  • People-centered — fostering personal choice by educating and empowering individuals to navigate and connect with resources
